Avances en el diagnóstico y manejo oportuno de las exantematosas de la primera infancia
Resumen
Introducción: La palabra exantema es un término compuesto del prefijo griego “ex” (afuera), “anthein” (florecer) y el sufijo “ema” (proceso), lo cual hace referencia a la erupción cutánea. Esta manifestación que está asociada a una reacción adversa o alérgica secundaria a medicamentos o a una enfermedad, juega un papel relevante en cuanto al reconocimiento de la patología, sin embargo, en la mayoría de los casos el exantema, no suele ser un síntoma patognomónico, por lo cual es importante conocer la historia clínica completa del paciente y además realizar un examen físico detallado teniendo en cuenta las características del mismo (4, 20). Estos últimos son aspectos relevantes para su reconocimiento, no obstante, en ocasiones se requiere la ayuda de ciertas pruebas de laboratorio o de la toma de una biopsia para realizar un diagnóstico preciso (1-3), por lo tanto, el manejo para cada una de ellas, será diferente y deberá ser de carácter oportuno para evitar complicaciones secundarias (5).
Objetivo: Identificar los avances en el diagnóstico y tratamiento de las enfermedades exantemáticas en la primera infancia.
Método: Se realizó una búsqueda sistemática con términos Mesh, usando bases de datos tales como Pubmed, Ovid, Cochrane, ClinicalKey, Google Scholar, UpToDate y ScienceDirect, de artículos publicados desde el mes de mayo del año 2019 hasta la fecha. Se encontraron una amplia variedad de artículos dentro de los que se hallaron revisiones sistemáticas de la literatura y metaanálisis, reporte de casos, estudios retrospectivos, estudios multicéntricos y revisiones bibliográficas, de manera que se evaluaron 120 artículos y se seleccionaron 46 de ellos, que incluían algoritmos diagnósticos, el manejo convencional de las enfermedades exantemáticas comunes en la primera infancia, además de nuevas propuestas para el diagnóstico y métodos recientes de abordaje para cada enfermedad en particular.
